Couldn't-verify

Couldn't-verify is an explicit bucket for claims a review leaned on but did not check, plus the blind spots no reviewer could see. It is the main guard against a confident, unanimous, wrong answer, because several models can converge on the same missing fact. Its sharper cousin: a verified citation means the receipt resolves — the cited line exists and the quoted text is there — not that the inference drawn from it is sound. The gate catches fabrication, not grounded-but-wrong reasoning.
